You can complete your application by following the checklist below.

Section A

1. Completed Application Form

2. A one-time, non-refundable application fee payable to AIMC Berkeley (credit card okay):

• $100 Standard
• $200 International and/or AOM Transfer
  

3. A Résumé covering your educational and professional background

4. A personal essay, 500 words in length, answering the following prompt: Describe your first experience with Acupuncture and Herbology, and describe the path that led you to choose Acupuncture & Oriental Medicine as a profession.

Section B


5. Official college and university transcripts

6. Two sealed letters of recommendation bearing the senders' signatures and contact information. Letters of recommendation should speak to an applicant's educational ability, professionalism, compassion, maturity, and/or overall ability to successfully complete the master's program and become a successful practitioner. (Faxes okay with phone call)

7. A completed Health Form, copy of TB test results, and verification of Hepatitis B immunization (or sign waiver in Health Form).

• Foreign transcript evaluation, if applicable
• Proof of English language proficiency, if applicable (diploma/degree verification or TOEFL/TSE results)
